When wanting to apply for or renew a Dutch driving licence, you can be asked to submit a CBR Declaration of Fitness/Suitability (Verklaring van Geschiktheid), which enables you to apply for a Dutch driving licence through your local municipality. This can also occur when you want the CBR to determine whether you are still fit enough to ride or drive with the driving licence that you already hold.

You can apply directly online yourself through MijnCBR.nl for immediate processing at € 41,00; you can also, at € 28,95 additional administration charges, request a hard copy through us, which we will then request the CBR to send to you by PostNL Registered surface mail.

!! You will then still as yet need to complete that form yourself and then post it to the CBR Medical Department; this procedure will take much longer and is more expensive!!

Furthermore, you may only ride or drive when you have been declared suitable to do so; this means that your mental and psychical fitness to drive has to be established. Applying for your own personal GV is done by submitting a CBR Statement of Health Form (Gezondheidsverklaring – GV for short), and is your first important step towards assessing your suitability to ride or drive.

On this GV you’ll need to self-certify your mental and psychical fitness to drive by answering questions about your general physical and mental condition, any disability, adverse medical condition, mental illness or any other defect or disorder so that the CBR Medical Department can determine whether you are immediately fit to drive, or whether a further medical examination will be required in order to determine your fitness.

These questions concern:

• Medications
• Arms and legs
• Vision and eyes
• Hart and arteries
• Diabetes mellitus
• Abuse of substances
• Reduced kidney functions
• Nervous system disorders
• Other complaints or disorders
• Lungs or blood conditions, transplants
• Behavioural disorders and psychiatric diagnosis
• Epilepsy, obstructive sleep and unconsciousness

and vary per ordered GV. The required GV depends on the driving licence categories that you are applying for.

There are three types of this GV

You can go to www.cbr.nl and settle this online yourself immediately, but when not having a DigID or not having a good command of the Dutch language – with all its specialized medical terminology – you can order your personal paper copy of this GV here at an additional fee. After receipt plus payment of your order, we will send this paper copy to you within 24 hours by surface mail and also by return send you a PDF file – fully in English – by e-mail with all the important information you’ll need to fill out this form. In this way, you can prepare yourself in advance for all the questions that will need to be answered!

This PDF file includes a full English translation of all the questions; where necessary, a specific question will also have a thorough explanation so that you can more easily and accurately answer with a ‘no’ or with a ‘yes’! It also includes a Driving Licence Chart, making it easier to understand which driving licence categories you can apply for.

Whether you’ll need to visit a general medical practitioner (GP or doctor) for supplementary information will depend on the answers given to these questions, and also how the CBR Medical Department interprets these. Note that only when you are over 75 years of age or a professional bus or heavy goods vehicle driver, you’ll always need to visit an examining medical practitioner afterwards. In those cases the CBR’s Medical Department will send you a form that needs to be completed by an examining doctor; where it’s immediately apparent that also information is needed from an additional doctor, like a specialist, you’ll also receive a referral to this doctor at the same time.
